Authorisation Letter

Eligible:

Passport have no longer be able to use (water damaged or lost)
Passport can't be extended.
Passport validity less than 6 months and won't be able to travel.
Wont be able to travel back to Cambodia to obtain a new passport in person. Third party "Friend, or relative" in Cambodia can obtain a new passport on behalf of you if they have this authorisation letter.

1. One(1) completed application form (Download Application Form)

2. Attach two(2) standard Australian passport photo

Recent at least six(6) months passport picture. Trim off white area of the photo .

3. Copy of Australian Visa

4. Copy of Cambodian Birth Certificate

5. Copy of Cambodian passport

Copy only Bio data page

6. Information of authorised person (Copy of Identification Card or Copy of Passport)

7. Fee (Payment shall be in Cash/Bank Cheque/Money Order)

You may pay one(1) cheque for multiple application.

8. 2 to 3 working days of processing time

9. Prepaid return (express) envelop

(Shipped back) Applicant need to provide a self address prepaid Express Envelope and be sure to retain tracking number.

Provide (shipped-back) prepaid return envelope that billed to your DHL account or credit card
DHL waybill slip
